  • How Do I Become A Patient?

    All first time patients are required by law to have a valid Arizona Medical Marijuana patient card and a secondary form of identification in order to purchase medicine. Secondary forms of identification include:

    • Valid Arizona Drivers License
    • Valid Arizona Identification Card
    • Valid Passport
    • Out of State Drivers License with proof of AZ Residency
    • Valid Residency Card for Arizona
    • Military Card with proof of Arizona Residency

  • How Do I Obtain an MMJ Card?

    Rules and Regulations to Obtain a Medical Marijuana Card in the State of Arizona:

    • Must be a current resident of Arizona
    • Must meet with a Licensed Physician in Arizona to get Legally Certified for a Medical Marijuana Card
    • Must have Current Medical Records Documenting a Qualifying Condition

Qualifying Conditions

  • What are the Qualifiying Conditions for a Medical Marijuana Card?

    Arizona Revised Statue ยง 36-2801 lists the following as Qualified Conditions:

    • Cancer
    • Glaucoma
    • Positive Status for Human Immunodeficiency Virus
    • Acquired Immune Deficiency Syndrome
    • Hepatitis C
    • Amyotrophic Lateral Sclerosis
    • Crohn's Disease
    • Alzheimer's Disease
    • Cachexia or Wasting Syndrome
    • Severe and Chronic Pain
    • Severe Nausea
    • Seizures
    • Epilepsy
    • Severe and Persistent Muscle Spasms
    • Multiple Sclerosis
    • Any other medical condition or its treatment added by the department pursuant to section 36-2801.01
